Adult Wellness
$350 per year
Our adult membership plan is available for patients 15 years of age and older who do not have periodontal disease.
Benefits:
2 routine dental cleanings per year
2 checkup exams per year
2 fluoride treatments per year
2 oral cancer screenings per year
1 annual wellness scan
Complimentary X-rays*
*Includes all routine intraoral X-rays and panoramic X-rays every 3 years
Discounted Services: Members receive a 20% discount on all dental procedures, including fillings, crowns, implants, teeth whitening, emergency dental exams, CBCT scans, and more.
No Waiting Periods: There are no waiting periods or pre-authorization requirements, no deductibles, and no maximums. You can begin using your membership benefits immediately upon enrollment.
Child Wellness
$250 per year
Our child membership plan is available for pediatric patients 14 years of age or younger.

$550 per year
Our periodontal maintenance plan is available for patients over the age of 14 who need periodontal disease treatment.

4 periodontal maintenance cleanings per year
Annual wellness scan

Oral cancer screening at each cleaning appointment
*Includes all routine intraoral X-rays and panoramic X-rays every 3 years

Family Bundling Options
Couple Wellness Plan: $650 per year
Family Wellness Plan (up to 4 members): $1,000 per year
Additional Family Members: $275 per year (adult); $175 per year (child)
